Lamar Odom has checked into rehab, RadarOnline.com has learned.

Just days after his divorce from Khloe Kardashian became final, the troubled former NBA star entered a facility in San Diego, E! News reported.

"It was his own decision. He wanted to work on his mental and physical health for the next chapter of his life," a source told E! News. "He has not fallen off the wagon, but he understands the importance of maintaining his sobriety."

In recent weeks, insiders told Radar that Odom had seemed to be on the mend.

"He's doing a lot better," a source told Radar. "He's spending time with his family."

Still, the source said at the time, "The temptations are there for him to fail," noting his family was still afraid.

Indeed, the last year has been a quick ride to rock bottom for Odom, 37.

He overdosed and nearly died at Nevada's Love Ranch brothel in October 2015.

But even that wasn't enough to slow his addiction.

Odom followed weeks of intense hospitalized recovery with bouts of public drinking and strip club visits — seemingly determined to drink and drug himself to death.

In a shocking video, Radar even caught him smoking a suspicious cigarette on a street corner in the middle of the day.

But he'd seemed to have turned a corner when he was spotted looking happy and healthy with his ex, Liza Morales, and kids Destiny, 18, and Lamar Jr., 14, at Disneyland over Thanksgiving weekend.

Story developing.
