Pregnancy can be an overwhelming experience at the best of times—even more so when the mom-to-be is only 14.

TLC‘s My Teen Is Pregnant And So Am I follows the experiences of four expectant teens, and their own moms who are also expecting, throughout the trials and tribulations of their pregnancies—and OKMagazine.com has a heartbreaking clip from Tuesday night’s upcoming episode.

In the clip, Saniyah, 14, is seen visibly struggling to come to terms with the fact that she is going to be giving birth soon, as her mother, Lemhele, 34, and her therapist try to prepare her for what’s ahead.

“She’s not looking at the fact she’s going to have a baby soon,” Lemhele explains, as her daughter looks blankly off to the side. “This whole thing isn’t registering yet.

“She’s going to become a mom—and she has to experience stuff herself because there’s not much I can do for her.”

As mom tends to her own newborn baby, Saniyah continues to stare off into space seemingly unable to engage in what is going on around her—including the fact that she is heavily pregnant.

“The truth is she’s a mom now,” the teen’s therapist says, attempting to draw her client into the reality of her situation. “That baby—that baby girl in your belly—that’s your daughter.”

It all becomes too much for Saniyah and she breaks down in tears—as mom tries to stop the crying and encourage her to talk instead—something the therapist discourages.

“It’s OK, she can cry,” she says. “She’s feeling. Look I know you don’t want to be here, you don’t want any of this to be happening and I understand that—it feels like it’s too much to think about, right?

“But, the situation is that it is happening—it’s a reality as hard as it is to consider. The reason why I’m here is to help you prepare for what’s to come, not just practically but emotionally.”

The therapist’s words seem to help mom just as much as Saniyah.

“Saniyah does not know what’s going on or what’s in store or what’s ahead for her,” Lemhele says. “I feel so bad looking at her because she’s so inexperienced, but I’m her mom so I have to try to be the best that I can and support her and tell her whatever it is today and all you feel today…. it’s going to get better, because at the end of the day there’s a human being in there and all kids are a blessing, so it’s going to be OK.”
