An LAPD officer, Ray Brown, told OK! that they received a call at around 11:37 am of a possible OD of a 25-year old in Hollywood. Demi was transported from her Hollywood Hills home, shown here.

Last month, the singer revealed that she had relapsed after being clean for over 6 years.  Her song, "Sober" divulged this information and she apologized to her fans for not staying sober.
