Sources confirm to OK! that Demi Moore was rushed to the hospital for substance abuse.
The actress has now been moved to a treatment facility, a source also confirms to OK!.

An insider tells OK!, "We are all worried about Demi. Her children really were the ones who have been the most concerned. She's been on a downward spiral ever since news broke of Ashton cheating on her with the barely legal girl."
