Sources confirm to OK! that Demi Moore was rushed to the hospital for substance abuse.
The actress has now been moved to a treatment facility, a source also confirms to OK!.

An insider tells OK!, "We are all worried about Demi. Her children really were the ones who have been the most concerned. She's been on a downward spiral ever since news broke of Ashton cheating on her with the barely legal girl."

Demi has been noticeable thin since reports of husband Ashton Kutcher's infidelity came out and led to the inevitable end of their marriage.
The star's rep tells OK!, "Because of the stresses in her life right now, Demi has chosen to seek professional assistance to treat her exhaustion and improve her overall health. She looks forward to getting well and is grateful for the support of her family and friends."
