They've been together since 2004 and tied the knot four years ago, but as she prepares to give birth to her fourth child, Heidi Klum has finally decided to take her husband Seal's last name.
The German-born host of Project Runway, 36, filed documents in the L.A County Superior Court Monday to change her name to Samuel.
The "Kiss From a Rose" singer's full name is Seal Henry Olusegun Olumide Adeola Samuel. Heidi will find out about the name-change on Nov. 20.

Former supermodel Heidi is due this month with her fourth child, a girl. She and Seal, 46, are parents to Henry, 3, Johan, 2, and Seal is the adopted dad of the blonde beauty's daughter Leni, 5.
