Lindsay Lohan has been taking a daily cocktail of prescription drugs "as long as she can remember," according to her latest probation report.
The report reveals that she has prescriptions for the antidepressants Zoloft and trazodone, the stimulent Adderall, Nexium for acid reflux and occasionally dilaudid for dental pain, says E! Online.

"Lohan states she wants to move on with her life, and feels that she has done everything that the court has asked her to do," wrote Chief Probation Officer Donald H. Blevins in his report, filed Tuesday in L.A. Superior Court.
Blevins states that Lindsay has tested negative for alcohol and drugs (other than her prescriptions) five times between May 27 and June 24, and reported for supervision as instructed.

"Her attitude has been positive," the report states, plus she's been "cooperative" with her probation officer and has submitted to drug tests when asked.

He closes the report by saying, "it is recommended that the defendant be continued on summary probation", but a judge upped it to formal probation, sentencing the actress to 90 days behind bars, starting no later than July 20, followed by 90 days of inpatient rehab.
