On Modern Family, Cam and Mitch adopted Lily from Vietnam. And a couple of seasons ago, they talked about potentially adopting a little boy, too. Lily needs a little bro, guys. Get on that.
Rachel on Glee
Lea Michele's character on Glee was adopted by her two dads as a baby. And do you remember who her birth mother is?
Beth on Glee
Rachel's birth mom is Shelby, the substitue teacher at McKinley who decided later in life that she wanted a daughter. So, she adopted Puck and Quinn's baby girl, Beth, at birth. Full circle, guys.
Dixon on 90210

And speaking of foster care, pretty much all the kids on the ABC Family show The Fosters went from living in foster care to living with adoptive families. Happy stories all around.
And one last ABC Family that deals heavily with foster care and adoption is the PLL spinoff, Ravenswood, which got canceled after its first season. The two main characters—Caleb and Miranda—end up spending their whole lives in foster care separately before meeting each other and becoming friends in that creepy town in Pennsylvania.
In the final episode of season 1 of CBS' Mom, 16-year-old Violet gives birth and makes the tough decision to give up her daughter to a couple who can't have kids of their own. It was a heartfelt and perfect ending to the season.
