Former First Lady Dr. Jill Biden shared candid reflections during an interview with People, opening up about her emotional journey following former President Joe Biden's diagnosis of stage 4 prostate cancer.
“Of course I’m scared,” Jill said. “I mean, first of all, he’s almost 10 years older than I am. And with an illness ... you kinda can’t let your head go there.”
The former president's office announced in May that he was diagnosed with aggressive grade group 5 prostate cancer, which has since metastasized to his bones.
'They're Going to Die'

The 75-year-old former FLOTUS explained that she "can't imagine life without" her husband of nearly five decades. However, rather than letting the weight of the illness overwhelm them daily, they choose to see the glass half full.
“It’s like, you can’t imagine life without them. But you know they’re going to die — just like I know I’m going to die,” she continued. “So part of it, you accept.”
The couple has made a conscious decision to focus on one another during this time.

Joe Biden is still keeping busy, according to his wife.
“We can’t look at life that way ... We look at it like, ‘Joe has cancer. He’s gonna die, so let’s make the most of every moment,’” Jill said.
"‘Let’s be together ... let’s find things that bring us joy. Let’s make this a positive journey’ — I mean, not a negative journey, because how can you live with that every day?” she added. “You can’t. It’s too heavy.”
She noted that while the combination of age, medications and treatments has slowed him down, the former POTUS still maintains a busy schedule of public appearances and travel.

'Strength of Our Family'

Beau Biden passed away from brain cancer in 2015.
“He’s always been the strength of our family,” Jill said.
Jill shared that she relies on her faith, praying every morning to their late son, Beau Biden, who passed away from brain cancer in 2015.
The Biden family's public vulnerability regarding the final chapters of life has prompted widespread support and brought fresh attention to the emotional realities of cancer caregiving.

Joe Biden revealed his diagnosis in May 2025.
“I think Joe will live with cancer ‘til the rest of his life,” Jill said.
In one of his major recent public appearances, Joe spoke at his alma mater, Syracuse University College of Law, for the unveiling of his official portrait.
Reflecting on his career, he delivered a speech on legacy and American institutions, stating, "Whatever my legacy may be, I hope that it will be said that I never stopped striving for the cause of democracy."
