Nicole Kidman May Have Undergone 'Subtle' Cosmetic Work as Critics Accuse Her of Looking 'Unrecognizable' in Latest Photoshoot, Plastic Surgeon Claims

An expert weighed in on fan feedback after new photos of Nicole Kidman prompted accusations that the actress appeared AI-generated.

Kidman, 59, modeled on the cover of British Vogue while promoting her upcoming film Practical Magic 2.

Fans accused the actress of looking "unrecognizable" and "AI-generated" in the images, prompting a medical explanation from plastic surgeon Dr. Omar Tillo.

"When people describe a face as looking ‘AI-generated’, they are often responding to a combination of features that feel almost too polished," he explained. "AI imagery tends to produce extremely smooth skin, very clean contours and a high degree of facial symmetry, so when we see those same characteristics in a real photograph, it can create that slightly uncanny impression."

In the images, Kidman is leaning against a wall with bleached eyebrows and high-contrast makeup as she stares directly into the camera.

According to the medical professional, her airbrushed-looking complexion and youthful glow could have been the result of subtle procedures, rather than an artificially generated image of Kidman.

"She may well have had maintenance work including Botox, subtle dermal fillers to maintain volume through the cheeks and mid-face and non-surgical skin-tightening or resurfacing treatments to help maintain skin firmness and texture," Tillo explained.

"If aesthetic treatments are contributing to what we’re seeing, subtle injectables and skin-quality or tightening treatments would be more plausible explanations than assuming she has undergone major surgery, with the dramatic difference in this particular shoot likely amplified considerably by the make-up, lighting and post-production," he added.

Tillo assessed that new photos of Kidman showed "strong definition" in her eyebrows, cheeks and jawbone, which could be due to the meticulously placed editing style of such a high-profile cover shoot.

"Styling and make-up can amplify this further by changing the brows, eye shape and perceived proportions of the face enough that somebody very familiar suddenly becomes harder to recognise," he stated.

The doctor admitted that assessments of her unrecognizable appearance were prompted by evidence of a "uniform" look that fans are growing more and more sensitive to due to the rise of artificial intelligence.

"The complexion appears exceptionally uniform, with very little of the natural skin texture, fine lines, pores and subtle tonal variation that we subconsciously expect to see in a close-up portrait," Tillo assessed. "Removing or softening those tiny details can make the skin appear more synthetic, particularly when combined with strong studio lighting."

Tillo elaborated that rumors of cosmetic work and editing were far more plausible than an AI version of the actress being used for the esteemed magazine's cover, but he didn't rule it out.

"None of this means that AI has necessarily been used, and it would be impossible to determine that simply by looking at the finished photograph," he pointed out. "Professional lighting, makeup, lens choice, colour grading and conventional retouching can dramatically alter how a face appears."

This cover is the second time in Kidman's recent press run that she has been caught in AI allegations. After the poster was released for Practical Magic 2, which hits theaters in September, fans had similar comments.

"Thought it was fan-made with AI," one fan said of the poster, which features Kidman and Sandra Bullock's side profiles, as another described it as "airbrushed."

"What I find particularly interesting is that audiences are becoming much more sensitive to this aesthetic. We now see AI-generated faces so frequently that people have developed an instinct for the visual cues associated with them," Tillo said of the reactions. "When a real image removes too much of the natural irregularity and texture that makes a face look human, people can recognise that something feels different even if they can’t immediately explain why."
